I, the undersigned, hereby acknowledge that participation in the ASEDA Academy Soccer Camp (the “Camp”) involves inherent risks, including but not limited to physical injury or harm due to accidents, contact with other players, physical exertion, and other activities associated with the Camp. I understand and voluntarily assume all such risks involved in the Camp, whether caused by the actions or inactions of myself, other participants, staff, or third parties.

I, for myself, my child (if under 18), my heirs, assigns, and legal representatives, hereby release, discharge, and hold harmless ASEDA, its officers, employees, volunteers, agents, and representatives from any and all claims, liabilities, damages, or costs arising out of or related to any injury, death, or damage sustained during participation in the Camp.

I understand that ASEDA does not provide medical insurance and that I am responsible for any medical costs that may arise from participation in the Camp, including any injury, illness, or emergency care needed. In the event of an emergency, I give permission for camp staff to seek medical treatment for myself or my child if necessary.

I affirm that the participant is in good health and physically able to participate in the activities of the Camp. I agree to notify the Camp organizers if any changes in health or medical status occur during the Camp.

I also grant permission to ASEDA to use any photographs, videos, or likenesses of the participant taken during the Camp for promotional or educational purposes, including but not limited to on websites, brochures, or social media. I understand that no compensation will be provided for such use.

Acknowledgment of Risk and Consent:

By signing below, I acknowledge that I have read and understood the above waiver and release of liability, and I voluntarily agree to all terms and conditions contained herein. I am aware that I am waiving certain legal rights, including the right to sue.
